Referral Process
University Pointe Pain Management Center 7759 University Drive Suite C West Chester, Ohio 45069 Ph: (513)475-8282 Fax: (513)475-8283 Clinic Hours Monday- 8:00-5:00 Tuesday- 8:00-5:00 Wednesday- 9:00-6:00 Thursday- 8:00-5:00 Friday- 8:00-5:00		Referral Process to see Dr Munir or Dr Akbik at the University Pointe Pain Center is initiated by your primary care doctor or your referring doctor. Your primary care doctor's office may call 513-475-7545 to schedule an appointment. Our intake coordinator will request the information needed to schedule an appointment. This may include but not limited to: 1. MRI reports, EMG results, X-ray and CT scan reports. 2. Most recent clinic notes. 3. Any specific question or reason for consultation (e.g. Vertebroplasty for osteoprotic painful fracture, epidural injection for sciatica or a consultation for medical management and optimization of medical management) 4. If seen by other pain doctors before, reports from their office or previous pain treatments that are utilized are also requested. After having received all needed information, our intake coordinator will call you and schedule an appointment with Drs Munir or Akbik. Please bring the following to your scheduled appointment MRI Films X-Rays Reports List of all Medications that you are currently taking Your insurance cards Names and addresses of referring and primary doctors Completed patient information forms Referral Forms and Instructions Referral Instructions Sheet Patient information forms Part 1 Patient information forms Part 2
